When people think of Head Boy, they think of people that are smart. Percy Weasley or someone who abides by the rules comes to mind. No one ever thinks of someone who was a Death Eater, on the wrong side of the war and is a renegade. Can Hermione see through all of that to the very core of Draco Malfoy?
