Nelson was just an ordinary guy, having recently moved from America, when a close shave with death at the park and then with his psychiatrist has him convinced that the man called "the Doctor" is on the adventure of a lifetime! (This is essentially another season between season 4 and the season 4 specials, plus some setup. Mostly canon-compliant, completely original adventures.)
