Two things JK did wrong. 1. Underpowered the brains of Death Eaters. 2. Allowed Harry to get away from the Dursleys unscathed. Harry never went to school. Never saw a human face except for the Dursleys. Never played, laughed, or smiled. Hermione was a mere target. Naive, scared, and innocent. And her mind an open target.
